The first Sentinel Hammer upgrade is in the World Spear level, which is the first level. After riding the dragon, the game will eventually funnel players through a narrow room with explosive barrels. Enemies pour in from both sides.

After that, do some light platforming. Jump across the pit with the flames shooting out of the walls.

The Escalation Encounter demon blob will be on the other side. Walk up to it and press the melee button to start the challenge. The hammer upgrade will unlock once every demon is dead.

The third and final Sentinel Hammer upgrade is very easy to find as it is right in front of the player in one of the early parts of the level. There will be the above gap after some of the tentacles pop out of the ground just after the Revenant. Jump across the gap and punch the last one to start the final Escalation Encounter.
