The hidden Doom Eternal: The Ancients Gods Part 2 milestones are for beating each of the DLC levels on Nightmare difficulty. This is the second hardest difficulty in the game. Beating the World Spear unlocks two of the weapon skins and the same applies for the other three levels in the game. Each just unlocks other skins for other weapons. The final one appears to unlock a podium and couple player icons, which are more valuable in multiplayer.
If some milestones are not showing up as completed, don’t worry. This has been a bug in the game since its launch as it takes completed milestones and still shows them as locked. Players still get to keep the rewards so this is merely a visual issue and not an unlocking issue.
Nightmare is pretty hard, but players do get to respawn, making this a decent challenge for those who want a hardcore experience without adding permadeath into the mix. The DLC is not too long, as well, making this a relatively small commitment. The Ancient Gods Part 1 did have a similar set of hidden milestones, but with different rewards, making them a little less desirable. Weapon skins are a more attractive reward than victory poses or podiums.
There is one milestone for beating the game on Ultra Nightmare, which is very tough and must be completed in one go without dying. Doing this unlocks the coveted Skullface Slayer skin. Thankfully, the Barbarian skin is not as hard to get.
