To get the secret ending in Cyberpunk 2077, you need to refrain from making a choice during the Nocturne OP55N1 mission. You can only unlock the hidden ending if you max out your relationship with Johnny, then wait long enough for him to offer an alternative choice.

We’ve tried to provide the basics above without giving away any plot spoilers. However, in order to elaborate on how the secret ending unlocks, we’ll have to dive into spoiler territory. Spoiler alert: The rest of this guide touches on several important details about the end of Cyberpunk 2077. Reader discretion is advised.

Throughout the course of the game, you’ll have several opportunities to appease Johnny Silverhand. The relationship between you two will ultimately determine which endings you’re eligible to receive. However, in order to get the secret ending, you’ll have to have at least formed a good relationship with him. Hopefully you’ve been playing it cool so far.

When the final mission Nocturne OP55N1 comes around, Johnny will offer a few choices. Most of them involve taking Omega Blockers or Pseudoendotrizine. Instead of choosing to take pills, wait at the decision screen for several minutes. Eventually, Johnny will propose a new option: A full-on suicide attack on Asakana. This will unlock the (Don’t Fear) The Reaper mission, one of the most difficult challenges in all of Cyberpunk 2077.

That’s right, the only way to get the secret ending in Cyberpunk 2077 is to avoid a decision that would lead you to any other ending. When Johnny asks what you want to do, simply wait until he comes up with a new approach. What happens after that is best left for you to discover.
